Download and complete our claim form.

Step 2

Email our team at [email protected] and attach your form with supporting documentation (detailed on page 1).

Step 3

You will receive an email acknowledgement and a claims specialist will be in touch with next steps within 24 to 48 hours (Monday to Friday).
